We are using a new online service called Register with a GP Survey that makes it easy to register with a GP Surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one. You can download the form here (PDF, 229KB).
You will need to bring a form of photographic ID (e.g. passport or driving licence) with your completed form.
The practice needs to record all your details including your NHS number if you know it. Your medical records will then be sent from your previous GP. Once registered you can see either of the GPs for your future appointments.
To register we will need the form completing in full for each member of your household. Please bring to surgery 2 forms of ID if possible. If you do not have ID because you are homeless please still speak to the staff at the Surgery.
